Registration forms are required for each child to provide necessary medical information and emergency contact numbers.

Cost:

Although Mom's Day Out is an outreach of Church at the Heights, it is not funded by the church. This is a self-supported program which operates from income generated through daily fees. Payment is required each day at pick up. If you are in need of child care on a regular basis, you can arrange for weekly payments.

• Please make checks payable to: Salem Ministries
• 0-1 years old $10 per hour
• 1 and older is $7 per hour
• any aditional children in family $5 per hour
• $14 minimum fee
• 10.00 fee per day for late payment

Reservations:

Please call 724-935-2645 to reserve a time for your child. When leaving a message, please include the following information:

• parent's name
• phone number
• reservation date(s)
• name(s) & age(s) of child(ren)
• All reservations and cancellations must be made by 10pm the night before

Drop-Off Procedure:

You may drop off your child provided space is available for that day. Non-reserved spaces are available on a first-come, first-serve basis only.
